  • On non-weekday BI EVE, you are responsible for: 
    • All diagnostic exams "Marked as Verified" 15 minutes before your NF/weekend shift time ends
    • Ultrasounds ordered 30 minutes before your shift time ends
    • For example, on BI Weekend shift you are responsible for studies up to 6:45 pm
  • Uber pool 9p-5a
    • Out of state not covered
    • Submit reimbursements to Maritza pending an Uber account

US TECH HOURS

Beth Israel

Mondays & Tuesday: 17 hour coverage (12 am to 7 am is uncovered)
Wednesday - Friday: 24 hour coverage
Weekends: 22 hour coverage (7 am to 9 am is uncovered)

BI ED US Responsibilities

BETH ISRAEL:
  • DVT (upper & lower extremities)
  • Gallbladder
  • Pelvic (Abdominal & Transvaginal)
  • Scrotum 
  • Peds --intussusception and appendicitis
If requested to perform ultrasound outside of what's delineated, contact & discuss with supervising radiologist.

If requested to perform ultrasound, always have a chaperone.


BI ON-CALL SCHEDULES


Managing Overnight MRI:

NO overnight BI MRI Tech for now.

If there is an emergent case that cannot wait until the morning & a CT can't be performed in the meantime, patient will need to be transferred to a different site. 

If this comes up, you can suggest that they contact Department Chair and Harold to discuss re-instate the MRI Tech on-call.





BI-SPECIFIC CALL POLICIES

Sites/Locations Covered: 
  • BI (Petrie)
  • KH (BI Brooklyn)
  • KD (Kingsway Diagnostic): 
    • CT and MR from KD do not need to be read unless the office calls for something urgent and they need a final attending read
  • CityMD: 
    • These patients are registered as Petrie or (rarely) PACC outpatients 
  • PUC (PACC/DUS Urgent care)
  • NYEE (NY Eye & Ear):
    • XR/CT do not need to be read unless it is emergent and in that case they would call the reading room
    • NYEE patients needing an MRI are sent to Petrie - these need to be read, and an attending needs to at least consult if not finalize; there are 2 classes of NYEEI MRI: those going to the Petrie ED and those getting a roundtrip transfer via the transfer service:
      • The ED cases are handled as any other ED case
      • Please see the roundtrip flowchart for the protocol regarding roundtrip transfers
      • Please see explanation of steps to follow for NYEE roundtrip transfers
  • SV (Stuyvesent Town Urgent Care): 
    •  Dictate all SV studies except for Dr. Arnouk's cases
  • NYRA (34th Street), Hudson Yards, One Medical, Credit Suisse: 
    • Occasional STAT read at the end of the day for which they will call the reading room
EMERGENCY ULTRASOUNDS

TURN OFF THE MACHINE AFTER STUDIES!

CLEANING PROBES: 
  1. Wipe all US probes to remove gel residue after use. 
  2. Wipe transvaginal probe to remove gel residue, and place the probe inside the same clear plastic bag with the MRN, so the techs can register  the correct information in the trophon log book while disinfecting the dirty probes the next morning. 
  3. Probes may need to be cleaned by resident if there is a high volume of TV US in one night.

PERFORM ON CALL EXAMS IN THE ED, EXCEPT:
  • If two exams will be performed at once (one in the ED and one upstairs), the charge nurse and ordering MD/PA should be in agreement
  • If the ED US machine is broken, inform the charge nurse so everyone is aware that ultrasounds will be performed upstairs the rest of the night
  • For any ultrasound related issues overnight, please e-mail the chiefs and Andrea at Lediana.Alzate-nehme@mountsinai.org

ED & IP NUCLEAR MEDICINE

Any MSBI ED or IP NM exam that MUST be performed will be done at West, either via a one-way transfer or round-trip if feasible. 
  • Gather clinical information from requesting physician
  • Review case with NM Attending & discuss NM study indication
  • Once approved by NM Attending, contact Raj Head NM Tech at West (ext. 23-6366) during business hours or West NM Tech on-call & give patient information and requested NM study
  • Coordinate roundtrip or inpatient transfer between Beth Israel & West. 
  • Refer to MSBI-to-MSW Nuclear Medicine PDF for step-by-step instructions
During COVID Era, ventilation is temporarily not being performed in V/Q scans. It is important to check the most recent Chest X-Ray for any abnormalities. If there are chest abnormalities, perfusion study will provide limited information. Be sure to notify NM attending of any chest abnormalities on the most recent Chest X-Ray when discussing the case.

Per Dr. Zeifer (8/7/2019): 
  1. In the BI ED, the need for labs can be waived by the radiology attending or the ED provider.
  2. Can be verbal to techwritten in comments section of the order, or note documented in emstat.
  3. No informed consent is required.
